Service Coordinator Assistant-LVN
Responsible for the coordination of the admissions and re-admissions. Identifies and implements resolution for a wide variety of problems related to resident admission. To act as a liaison between the resident, family members, the interdisciplinary team and community to assure that services to meet the resident's needs are initiated and met in accordance with their plans of care and treatment. /act as a community liaison with hospital staff, insurance companies and other payers, and physicians and other providers to notify and inform of services offered. Preparation and implementation of a discharge plan for residents return home or other appropriate placement. Participates in education on and implementation of clinical guidelines and polices. Provides or arranges patient teaching as appropriate.
Regulatory Requirements: Current Licensed Vocational Nurse (LVN) as issued by the State of California is mandatory at all times.
Current Basic Life Support certification BLS/CPR
Two or more years of previous experience in either an acute hospital or equivalent.
Pay: $22 to $25/hour
